The Trump administration’s push to bring US drug spending down is a powerful incentive for pharmaceutical companies to boost prices and reduce access elsewhere in the world, a study found.
The president wants drug prices on the US Medicare federal health insurance program to match those in other wealthy countries. Yet researchers found that for about three in four drugs they studied, companies would lose more revenue in the US by charging Medicare a lower price than their total sales from the drug in the cheaper reference country used to set the new benchmark.
As a result, drugmakers might raise prices in some countries or pull out of markets altogether, according to the study published Sunday in The Lancet.
Companies have cut a slew of deals with the White House, while some are also seeking to use the US policy as leverage to raise prices elsewhere. Astellas Pharma Inc., for example, said it got a higher price for a new eye medicine in Japan this year due to concerns over Trump’s pricing policy.

In Europe, some companies are scaling back plans. Biogen Inc. Chief Executive Officer Chris Viehbacher said this year the company would introduce its new postpartum depression drug Zurzuvae in only a few European countries, while Roche Holding AG has said it may never introduce an as-yet-unapproved breast cancer pill in its home market of Switzerland.
In the study, researchers from Brigham and Women’s Hospital analyzed 195 patented medicines that made up $87.9 billion of Medicare’s spending in 2024. They estimated what the program pays for the drugs after confidential rebates and discounts, and compared that to the 19 reference countries.
Lowering the Medicare price to match would save two pilot programs the US administration is using to test the policy $11.6 billion, the research team found. But that figure shrinks to $3.3 billion if the 17 companies that made separate pricing deals with the White House get an exemption.
Nine more drugmakers struck pricing deals with the administration earlier this month, which weren’t included in the analysis.
